You may be eligible for the Family Planning Benefit Program (FPBP). Planning a family is important. Birth control and other family planning services are available. FPBP is free to females and males.
What services can I get?
All forms of birth control like pills, condoms, IUDs, and the patch
Check-ups and Pap smears during a family planning visit
Emergency contraception like Plan B
HIV counseling and testing related to family planning visits
Information about family planning and choices for birth control
Laboratory tests related to family planning services
Sterilization
Who can get FPBP services?
If you do not qualify for or choose not to apply for Medicaid or Family Health Plus
If you are able to have children (males are eligible, too)
If you are a U.S. citizen, national, Native American or has satisfactory immigration status
If you live in New York State
If you meet income guidelines
What should I bring to my appointment?
Driver's license, permit, student id or sheriff's id
Insurance card or Medicaid card, if you have one
Latest month of pay stubs
Original birth certificate, or passport if possible
Proof of permanent address like report card, monthly bills, or magazine subscription
Your social security number
Sliding fee scale
Cost for services is determined by a sliding fee scale based on your income and the number of people you support. Many people qualify for free services.
Medicaid
Medicaid, Preferred Care Option, Blue Choice Option, Child Health Plus and Family Health Plus are accepted with no approval by your primary care provider needed.
